If you are a new patient to the Southwest Pain Clinic, you can make your visit smoother by coming prepared to your appointment.  

The following is a list of reminders of what to bring to your first visit with the doctor:

1) New Patient Forms

2) Photo ID

3) Insurance Card

4) Any medical records that you have: bloodwork, MRI, X-Rays, list of current medications etc.  While it isn't mandatory that you bring your medical records with you on your first visit, it does help the doctor in determining the type of treatment that will be provided.
